What is Uric Acid?

Uric acid is the end product of purine metabolism, generated when the body breaks down compounds found in both endogenous cells and dietary sources such as red meat, seafood, and alcohol. It is primarily produced in the liver and excreted through the kidneys and gastrointestinal tract.

While often associated with gout, uric acid also plays a dual role in physiology. At normal levels, it acts as a circulating antioxidant, helping neutralize free radicals. However, when levels rise beyond the body’s ability to excrete it, uric acid can crystallize in tissues and joints and contribute to systemic inflammation, oxidative stress, and metabolic dysfunction.
 

 

Why Test for Uric Acid?

Evaluating uric acid levels helps clinicians assess metabolic health, renal function, and inflammatory burden. It is commonly used to investigate:

• Gout and crystal-induced joint pain
• Chronic kidney disease or impaired renal clearance
• Metabolic syndrome and insulin resistance
• Hypertension and cardiovascular risk
• High purine intake or alcohol use
• Unexplained fatigue, inflammation, or joint discomfort

Uric acid is increasingly recognized as more than a gout marker—it can reflect broader metabolic and cardiometabolic patterns.

Clinical Significance of Uric Acid Levels

  • High Uric Acid Levels: Levels above ~6.8–7.0 mg/dL exceed the solubility threshold and increase the risk of crystal formation. Elevated uric acid is associated with gout, uric acid kidney stones, hypertension, endothelial dysfunction, increased cardiovascular event risk, insulin resistance, metabolic syndrome, and impaired renal clearance. Beyond these associations, uric acid serves as a marker of broader vascular and metabolic stress.

    In oncology settings, rapid cell turnover—such as in leukemia, lymphoma, or during chemotherapy—can lead to acute hyperuricemia and tumor lysis syndrome, a condition that may result in renal failure if not closely monitored and managed. In pregnancy, elevated uric acid levels may be observed in preeclampsia or eclampsia and can reflect underlying endothelial dysfunction and hypertensive complications, particularly in high-risk patients. Postmenopausal women may also exhibit increased uric acid levels, likely due to reduced estrogen-mediated excretion.

  • Low Uric Acid Levels: Serum uric acid levels below ~3.5 mg/dL may reflect reduced antioxidant capacity, liver dysfunction, certain genetic conditions, or increased renal excretion. Although less common, low levels may be observed in individuals with chronic illness, malnutrition, or as a result of medication effects, and may indicate diminished physiologic resilience in some contexts.

 

Actionable Insights

Uric acid should be interpreted within a broader metabolic, renal, and clinical context rather than as an isolated marker. Elevated levels often reflect upstream drivers such as insulin resistance, high fructose intake, alcohol use, medication effects, or reduced kidney function. Determining the underlying cause of elevation is clinically useful; a 24-hour urine uric acid collection can help differentiate between overproduction and underexcretion.

Medication review also plays an important role, as certain agents—including diuretics, low-dose aspirin, and niacin—can raise uric acid levels, while others such as losartan and fenofibrate may contribute to lowering them. Closer monitoring may be warranted in higher-risk populations, including oncology patients, individuals with renal impairment, and those with high-risk pregnancies. It is also worth noting that even high-normal uric acid levels may signal early metabolic or vascular dysfunction.

Clinical management often focuses on improving insulin sensitivity, dietary patterns, hydration status, and renal clearance rather than targeting uric acid in isolation. Monitoring trends over time can provide more meaningful insight into overall metabolic health and treatment response.

• Women: ~3.5–5.5 mg/dL
