What is Small Dense LDL (sdLDL)?
Small dense LDL (sdLDL) refers to a subtype of low-density lipoprotein particles that are smaller, denser, and more atherogenic than larger, buoyant LDL particles. While standard LDL-C measures the total cholesterol carried by LDL, it does not differentiate between particle size or density.
sdLDL particles are more likely to:
• Penetrate the arterial wall
• Undergo oxidation
• Remain in circulation longer
These properties make sdLDL significantly more associated with plaque formation and cardiovascular disease risk compared to larger LDL particles. Elevated sdLDL is commonly seen in insulin resistance, metabolic syndrome, and chronic inflammation.

Clinical Significance of sdLDL Levels
- High sdLDL Levels: sdLDL levels above ~30–40 mg/dL are generally considered elevated

enhanced ability to infiltrate arterial walls and undergo oxidation. High levels are commonly seen in
insulin resistance and type 2 diabetes, elevated triglycerides with low HDL, obesity and visceral
adiposity, as well as chronic inflammation. Clinically, this pattern is often linked to accelerated
atherosclerosis, endothelial dysfunction, and increased risk of coronary artery disease—even when
LDL-C appears within range.
- High sdLDL Levels: sdLDL levels below ~20–30 mg/dL are generally considered favorable and reflect a predominance of larger, less atherogenic LDL particles. This pattern is often associated with good
metabolic health, balanced lipid metabolism, and lower overall cardiovascular risk.

triglycerides, HDL, ApoB, and markers of insulin resistance. Elevated sdLDL often reflects an

HDL, visceral adiposity, and chronic inflammation—rather than a primary lipid disorder. This pattern
contributes to a more atherogenic environment, with increased susceptibility to lipoprotein oxidation,
endothelial dysfunction, and plaque development, even when LDL-C appears within range. In many
cases, addressing insulin resistance, dietary patterns (particularly refined carbohydrates),
inflammation, and body composition can significantly reduce sdLDL levels. Conversely, lower sdLDL
levels tend to reflect more favorable lipid distribution and metabolic balance. Monitoring trends over
time helps assess whether interventions are improving lipoprotein quality, not just cholesterol levels.
